What You'll Do Evaluate patients in their homes and develop individualized treatment plans. Improve mobility, strength, balance, endurance, and overall functional independence. Educate patients and caregivers on exercises, adaptive strategies, and injury prevention. Collaborate closely with nurses, physicians, occupational therapists, speech therapists, and the interdisciplinary care team. Monitor patient progress, modify treatment plans, and document outcomes accurately. Empower patients to remain safe, independent, and confident at home. What We're Looking For Active Minnesota Physical Therapist (PT) license. Strong clinical reasoning and excellent communication skills. Passion for helping patients maximize independence in their own homes. Ability to work independently while managing a field-based schedule.
