We're locally owned, clinician-focused, and committed to helping our nurses succeed both professionally and personally. What You'll Do Deliver skilled nursing visits throughout the Twin Cities Metro. Perform wound care, medication management, and patient assessments. Educate patients and caregivers on treatments and disease management. Communicate closely with RN Case Managers, physicians, and the interdisciplinary team. Complete accurate documentation using our electronic medical record. Help patients remain independent and avoid unnecessary hospitalizations. What We're Looking For Active Minnesota Licensed Practical Nurse (LPN) license. Strong communication and critical thinking skills. Comfortable working independently in a field-based role. Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred. Requirements

Must hold an active Minnesota Licensed Practical Nurse (LPN) license and possess a valid driver's license with reliable transportation. Previous experience in skilled nursing, rehab, or home health is preferred.
