Perform wound care, medication management, and patient assessments. Educate patients and caregivers on treatments and disease management. Communicate closely with RN Case Managers, physicians, and the interdisciplinary team. Complete accurate documentation using our electronic medical record. Help patients remain independent and avoid unnecessary hospitalizations. What We're Looking For Active Minnesota Licensed Practical Nurse (LPN) license. Strong communication and critical thinking skills. Comfortable working independently in a field-based role. Previous experience in skilled nursing, TCU, rehab, hospice, or home health is preferred.
