Improve mobility, strength, balance, endurance, and functional independence. Educate patients and caregivers to support long-term recovery. Collaborate with nurses, physicians, occupational therapists, speech therapists, and other members of the interdisciplinary team. Monitor progress and adjust treatment plans to maximize outcomes. Complete accurate and timely documentation. What We're Looking For Active Minnesota Physical Therapist (PT) license. Excellent clinical reasoning and communication skills. Passion for helping patients achieve meaningful, lasting outcomes. Ability to work independently in a field-based setting. Valid driver's license and reliable transportation.
