What You'll Do Evaluate patients in their homes and develop individualized occupational therapy treatment plans. Improve independence with activities of daily living, home safety, upper extremity function, cognition, and adaptive techniques. Recommend home modifications and adaptive equipment that support long-term independence. Educate patients and caregivers on strategies that improve safety and function. Collaborate closely with physicians and the interdisciplinary care team to achieve outstanding patient outcomes. Complete timely, accurate clinical documentation. What We're Looking For Active Minnesota Occupational Therapist (OTR/L) license. Strong clinical reasoning and patient-centered care philosophy. Excellent communication and organizational skills.
