Develop individualized treatment plans that improve independence with activities of daily living. Assess home safety and recommend adaptive equipment and environmental modifications. Educate patients and caregivers on techniques that improve safety, function, and quality of life. Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team. Complete timely, accurate documentation while monitoring patient progress. What We're Looking For Active Minnesota Occupational Therapist (OTR/L) license. Passion for helping patients remain independent at home. Strong communication, organization, and clinical reasoning skills. Home health experience is preferred but not required. Valid driver's license and reliable transportation. Requirements
