A collaborative, therapist-driven culture that values autonomy and clinical excellence. What You'll Do Complete comprehensive occupational therapy evaluations in patients' homes. Develop individualized treatment plans focused on ADLs, safety, upper extremity function, cognition, and independence. Recommend adaptive equipment, home modifications, and energy conservation strategies. Educate patients and caregivers to promote long-term success and reduce fall risk. Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team. Maintain accurate and timely documentation using our electronic medical record. What We're Looking For Active Minnesota Occupational Therapist (OTR/L) license. Strong clinical reasoning and patient-centered approach. Excellent communication and organizational skills. Ability to work independently while managing a field-based schedule. Home health experience is welcomed but not required. Valid driver's license and reliable transportation. About Home Health Care, Inc.
