Develop individualized treatment plans focused on ADLs, safety, upper extremity function, cognition, and independence. Recommend adaptive equipment, home modifications, and energy conservation strategies. Educate patients and caregivers to promote long-term success and reduce fall risk. Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team. Maintain accurate and timely documentation using our electronic medical record. What We're Looking For Active Minnesota Occupational Therapist (OTR/L) license. Strong clinical reasoning and patient-centered approach. Excellent communication and organizational skills. Ability to work independently while managing a field-based schedule.
