Flexible scheduling and manageable caseloads designed to support work-life balance. What You'll Do Carry out treatment plans established by the supervising Physical Therapist. Provide therapeutic exercises, gait training, balance training, transfer training, and functional mobility interventions. Monitor patient progress and communicate changes to the supervising PT. Educate patients and caregivers on exercises, safety techniques, and home programs. Collaborate with nurses, therapists, physicians, and the interdisciplinary care team. Complete accurate and timely documentation. What We're Looking For Active Minnesota Licensed Physical Therapist Assistant (PTA) license. Excellent communication and patient education skills. Passion for helping patients improve mobility and independence. Home health experience is preferred but not required. Valid driver's license and reliable transportation. About Home Health Care, Inc. Requirements

Must hold an active Minnesota Licensed Physical Therapist Assistant (PTA) license and a valid driver's license with reliable transportation. Excellent communication skills are required, and home health experience is preferred.
