  • What you’ll do
  • Provide in-home occupational therapy services to patients and, when applicable, in assisted living settings
  • Evaluate patients’ physical, cognitive, and environmental needs
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Help patients improve daily living skills, mobility, and overall independence
  • Recommend adaptive equipment and home modifications to enhance safety
  • Educate patients and caregivers on treatment plans, exercises, and long-term wellness strategies
  • Document patient progress and adjust care plans as needed
  • What we’re looking for
  • Active Occupational Therapist license in the State of New Hampshire
  • Graduate of an accredited Occupational Therapy program
  • Minimum of 2 years of clinical experience (home health preferred)
  • Current CPR certification
  • Strong clinical judgment, communication, and interpersonal skills
  • Ability to work independently and manage a flexible schedule
