As a Home Health Physical Therapist, you'll evaluate patients and develop individualized treatment plans to help them regain independence and improve mobility. You'll provide hands-on therapy, monitor progress, and collaborate with other healthcare professionals to coordinate care.

  • As a Home Health Physical Therapist, you'll help patients recover safely and confidently in their own homes by:
  • Evaluating patients and developing individualized treatment plans
  • Restoring strength, mobility, balance, and functional independence
  • Providing hands-on therapy and therapeutic exercise
  • Monitoring progress and adjusting care plans as needed
  • Collaborating with nurses, occupational therapists, speech therapists, home health aides, and other clinicians to coordinate care
  • Educating patients and family members on exercises, safety strategies, and ongoing recovery
  • Completing accurate and timely clinical documentation
  • What we're looking for
  • Graduate of an accredited Physical Therapy program
  • Active New Hampshire Physical Therapist license
  • Minimum of one year of PT experience (home health experience preferred)
  • Current CPR certification
  • Knowledge of state and federal home health regulations
  • Excellent clinical judgment, communication, and patient education skills
  • Compassionate, organized, and committed to delivering exceptional patient care

Candidates must be graduates of an accredited Physical Therapy program and hold an active New Hampshire Physical Therapist license. A minimum of one year of PT experience is required, with home health experience preferred.
