Assist with diagnostic evaluations such as specimen collection, laboratory processing, and basic imaging procedures when necessary. Collaborate with interdisciplinary teams to develop and implement individualized care plans, discharge planning, and follow-up care strategies. Maintain accurate medical documentation in compliance with HIPAA regulations and facility policies. Provide specialized care for patients with complex needs including dementia, Alzheimer's disease, memory care, or those requiring ventilator management or tube feeding. Ensure infection control practices are rigorously followed to prevent healthcare-associated infections across all patient populations. Qualifications Valid RN or LPN license in good standing with relevant state licensing boards. Prior experience in acute care settings such as hospitals (including ICU or emergency room), nursing homes, or long-term care facilities is highly desirable. Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; familiarity with ICD coding (ICD-9/10) is advantageous. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology necessary for accurate patient assessment and medication administration. Experience working with diverse patient populations including seniors, individuals with developmental disabilities, or those requiring specialized dementia or memory care services. Ability to respond effectively in high-pressure situations involving urgent care or trauma scenarios such as Level I or Level II trauma centers.
