The Clinical Care Specialist manages a caseload to provide case management, behavioral health counseling, and outreach services. They collaborate with professionals to link clients to community resources and support independent living to avoid restrictive arrangements.
Under the administrative supervision of a program director, and the clinical supervision of a team lead, the Clinical Care Specialist will: Maintains a caseload and conducts case management services for clinicians; Makes appropriate referrals to address mental health and substance use needs; Links clients and community resources effectively; Offers behavioral health counseling and outreach; Ensures clients attend scheduled appointments in the community and at Health Solutions; Identifies stressors and precursors that led to the current situation or crises; Identifies available coping mechanisms and natural supports; Enhances problem-solving, independent living, and socialization skills among clients; Provides support for clients to remain in home and community to avoid more restrictive living arrangements; Maintains accurate records in the agency’s electronic health record; and Performs other job-related duties assigned by the director or designee. Physical requirements include the following: Must be able to remain in a stationary position at least 50% of the time. Frequently communicates with physicians about medical notes, must be able to exchange accurate information in these situations. Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer. Occasionally lifts and carries items up to 20 pounds. Occasional exposure to communicable diseases and bodily fluids.
