Are you a compassionate Occupational Therapist who values patient-centered care and enjoys working independently? We are seeking a dedicated Home Health Occupational Therapist to join our growing team. In this role, you will help patients regain independence, improve safety, and achieve their functional goals in the place they call home.
As a member of our interdisciplinary team, you will have the opportunity to make a meaningful impact on patients' lives by providing skilled therapy services, caregiver education, and personalized treatment plans that support recovery and quality of life.
What you'll do
Perform comprehensive occupational therapy evaluations in patients' homes.
Develop individualized treatment plans based on physician orders and patient goals.
Provide skilled interventions to improve activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
Assess home safety and recommend adaptive equipment and environmental modifications.
Train patients and caregivers in energy conservation, fall prevention, and compensatory strategies.
Collaborate with nurses, physical therapists, speech therapists, social workers, physicians, and caregivers to coordinate patient care.
Monitor patient progress and adjust treatment plans as appropriate.
Complete timely and accurate electronic documentation.
Participate in interdisciplinary team meetings and case conferences.
Ensure compliance with all agency policies, payer requirements, and regulatory standards.
Required qualifications
Graduate of an accredited Occupational Therapy program.
Current Occupational Therapist license in the state of practice.
Current CPR certification.
Valid driver's license, reliable transportation, and current automobile insurance.
Strong communication, organizational, and clinical assessment skills.
