At Centerpoint Health in Winchester, H2 Health provides inpatient therapy services to a diverse adult and geriatric population.
In this role, you'll work with a diverse adult and geriatric inpatient population recovering from neurological conditions, stroke, orthopedic injuries, surgical procedures, medically complex illnesses, balance impairments, and functional mobility limitations.
You'll collaborate closely with physicians, nursing staff, occupational therapists, speech-language pathologists, and case management teams to help patients regain independence and achieve successful discharge outcomes.
Your Role: Perform comprehensive physical therapy evaluations assessing strength, mobility, balance, endurance, and functional performance.
Develop and implement individualized, evidence-based treatment plans.
Provide skilled interventions focused on mobility training, gait training, balance retraining, transfer training, and functional restoration.
Collaborate closely with physicians, nursing, case management, and OT/SLP teams.
Educate patients and caregivers on treatment plans, safety strategies, and discharge recommendations.
Complete timely and accurate documentation in accordance with hospital standards.
Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program.
Current Kentucky Physical Therapist license or eligibility for licensure.
CPR certification, where applicable.
Strong communication and interpersonal skills.
