As an LPN in Long-Term Care, you’ll deliver essential bedside and coordinated nursing care under supervision, ensuring residents receive attentive, comprehensive support tailored to their evolving needs. Key responsibilities include administering medications accurately and safely, monitoring vital signs and symptom changes, performing wound care, assisting with activities of daily living, and helping manage chronic conditions within a multidisciplinary team. You’ll participate in individualized care planning, document thoroughly in electronic medical records, and contribute to the development and adjustment of nursing interventions that promote safety, comfort, and independence. You’ll assist with preventative measures, infection control, fall prevention, respiratory support, and palliative care considerations while maintaining clear, compassionate communication with residents, families, and colleagues. The role also involves collaborating with RNs, CNAs, therapists, and social services to coordinate care transitions and ensure continuity across shifts and settings.
