The Masters Social Worker independently assesses needs, coordinates, and delivers social services for participants and families, participating in the Participant Care Team and Interdisciplinary Team for assessment, intervention, and management of social service needs. Duties include conducting comprehensive bio-psychosocial assessments, developing individualized treatment plans, providing counseling, managing issues like housing and safety, and coordinating care transitions.
Candidates must possess a Masters Social Worker degree from an accredited school and be licensed to practice in the State of Michigan, with a Limited License considered based on experience. A minimum of one year of clinical social work experience, preferably in geriatrics, is required, along with knowledge of therapeutic techniques and compliance with relevant laws.
