You’ll perform comprehensive assessments, write clear documentation, implement fall prevention strategies, and support discharge planning for residents moving between care settings. You’ll mentor CNAs and LPNs, model evidence-based practice, and contribute to quality improvement initiatives that reduce hospital readmissions.
