The RN Care Coordinator will collaborate with residents, families, and the Interdisciplinary Team to coordinate care and services during skilled stays, completing admission assessments to facilitate resource management and discharge planning. Responsibilities include developing individualized plans of care, identifying resident needs, and ensuring high-quality, cost-effective care while facilitating continuity post-discharge.
Candidates must possess an active Ohio RN license and a minimum of five years of skilled nursing experience within a long-term care setting. Required knowledge includes Medicare Part A, Managed Care insurance plans, and the functioning of the Interdisciplinary Team in skilled nursing facilities.
We need an RN Care Coordinator to join our team at The Advanced Therapy Center at Chesterwood Village in West Chester, Ohio.
$36 to $43 Per Hour
You'll collaborate with skilled residents, families, and the Interdisciplinary Team (nursing, therapy, social services, and case management) to coordinate care and services delivered during skilled stays (upon admission up to 30-days post-discharge).
You'll complete admission assessments with residents/family within 48 hours of admission in order to facilitate resource management, treatment plan formulation, and discharge planning. You'll also effectively communicate with residents, families, and the Interdisciplinary Team to promote patient-centered care and improve clinical outcomes.
