The role involves assisting the Licensed Social Worker (LSW) in delivering social services to residents and their families, focusing on adjustment, identifying needs, and resolving issues. Key duties include aiding in assessments (IPS and MDS), supporting discharge planning by arranging community resources, and attending Care Conferences.
Candidates must be approved by the LSW and facility administrator, and ideal applicants will be a STNA or eligible with prior experience in long-term care settings. Excellent communication skills are also required for this position.
To provide assistance to the LSW/Social Service Director in providing social services for residents and families. Salary $16.50-$28.00 based on experience.
