As a Case Manager, you are essential in providing coordinated, person-centered services for this court-mandated expansion. Your role supports AllHealth Network’s Intensive Outpatient Services (IOS) Division by filling gaps in care with specialized behavioral health services aligned with court requirements. The position emphasizes care coordination, resource navigation, and community support to ensure ongoing care, encourage recovery, and strengthen partnerships between AllHealth Network and judicial stakeholders.
What you’ll do
Maintain an active caseload of justice‑involved clients in accordance with department standards
Provide and document case management and care coordination services in compliance with regulatory and contractual requirements
Develop, implement, and update multidisciplinary care plans in collaboration with clients and the treatment team
Assist clients in identifying recovery goals and coordinating services and providers to support those goals
Act as a client advocate and liaison with internal teams, community providers, and criminal justice partners
Provide coaching and support services aligned with client readiness to change, including motivational interviewing, skills‑based training, and self‑management support
Facilitate referrals to community‑based organizations and resources aligned with identified client needs
Work collaboratively within an interdisciplinary, team‑based model, including telephonic and face‑to‑face contacts and community‑based visits
Travel as required for hospital visits, provider offices, community organizations, and home visits
Promote a positive, collaborative team culture within AllHealth Network
Positively contribute to the organization’s financial health through delivery of billable services and data‑informed program improvement

What you bring
Bachelor’s degree in a relevant clinical field such as Social Work, Clinical Psychology, or Counseling.
Minimum of two (2) years of related clinical experience serving diverse populations in behavioral health, physical health, or criminal justice settings.
Demonstrated experience in case management, care coordination, and community-based services.
Strong assessment, care planning, and clinical documentation skills.
Working knowledge of community resources, treatment options, funding sources, and support programs.
Proficiency with PC-based software systems, including Word, Excel, PowerPoint, and electronic systems.
Excellent written and verbal communication, time management, problem-solving, and decision-making skills.
Ability to work independently while contributing effectively within a multidisciplinary team environment.
Must possess reliable transportation, a valid driver’s license, a good driving record, and proof of insurance

$26 - $28 an hour
The base salary range represents the low and high end of the allhealth network salary range for this position. Actual salaries will vary and may be above or below the range based on various factors including but not limited to experience, education, training, merit, and the ability to embody the allhealth network mission and values. The range listed is just one component of allhealth networks’ total compensation package for employees. Other rewards may include short-Term and long-Term incentives as well as a generous benefits package detailed below.
