Care Coordinator – IHPA

Knowledge skills and abilities

• Excellent oral and written communication skills, including ability to access social media, to provide trainings and community presentations and to exhibit proficiency in Microsoft Office Suite and Google tools.
• Punctual, reliable and dependable with the ability to work flexible hours (some evenings and weekends).
• Ability to work effectively with diverse individuals and groups.
• Self-starter that is detail-oriented and thinks creatively and strategically.
• Ability to produce high-quality work independently.
• Demonstrated commitment to social justice, community empowerment, and advocacy

Qualifications

• One year of experience providing case management and/or community support services in the behavioral health field required.
• Bachelor’s degree in a Psychology, social work or human services related field required.
• Experience working with diverse clientele with behavioral health needs preferred.
• Knowledge of and/or experience with care coordination and Managed Care Organizations (MCO), preferred.
• Bilingual in Spanish preferred.

Additional requirements

• Satisfactorily pass all relevant screenings and criminal background checks.

Travel/Transportation requirements

• Driving for work required with transportation of clients in a personally owned vehicle.

• Physical demands
• While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
