The Care Coordinator provides outreach, engagement, and linkage to integrated services for members while monitoring population health platforms and hospital admission feeds. They also facilitate post-discharge appointments and provide education to staff and community providers regarding care coordination workflows.
Candidates must possess a Bachelor's degree in Psychology, Social Work, or a related human services field and at least one year of experience in case management or community support. Proficiency in Microsoft Office and Google tools is required, along with the ability to drive and transport clients.

The average starting salary for this position will fall in the range of $47,000 to $52,000 annually. Where candidates fall in this range will be based on skill and experience level.

• Driving for work required with transportation of clients in a personally owned vehicle.
