$18 - $20 / HOUR
Manage a full caseload of families in the Short-term Services Program to resolve concrete and referral needs within 90 days. Provide support through referrals, transportation, and assistance with applications in office, home, and community settings.
Requires an Associate's degree and one to two years of related experience or an equivalent combination of education and experience. A valid New York State driver's license is mandatory.
Manage a full caseload. Complete all required trainings, participate in supervision with supervisor and attend required meetings and in-services as directed by supervisor. Establish positive and professional relationships with families. Model and demonstrate problem-solving and decision-making skills. Work with SCDSS case managers to identify service needs of at-risk families. Have a thorough knowledge of community services in Schenectady County. Help families obtain services they need by making referrals, providing transportation, accompanying them to appointments, assisting with applications (medical, schools, DSS, etc.). Maintain good working relationships with DSS/Support Unit caseworkers. Complete progress notes or other case record notes promptly. Maintain cultural sensitivity, competence and client confidentiality. Other duties as assigned.
