• Per Diem Summary The Per Diem Physical Therapist provides skilled hospice care services to adult and geriatric patients across a variety of settings, including private homes, assisted living communities, and inpatient facilities.
• As a key member of the interdisciplinary care team, the Physical Therapist delivers patient- and family-centered care that promotes comfort, safety, mobility, and dignity at end of life.
• This role supports patients in remaining in their home setting whenever possible while receiving high-quality, compassionate care.

Qualifications

Minimum of one year of clinical experience as a licensed Physical Therapist required. Graduate of an accredited physical therapy program. Home health or hospice experience strongly preferred; or an equivalent combination of education and experience that provides the necessary skills to perform the role. Ability to work independently and manage time effectively in a field-based environment. Strong clinical assessment, problem-solving, and organizational skills. Excellent interpersonal and communication skills. Proficiency with electronic medical records (EMR) systems preferred. Valid driver’s license and reliable transportation required. #INDOTH The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post.
