Provide compassionate counseling, guidance, and emotional support to residents and families while helping them navigate transitions, care planning, and healthcare recommendations.
Assist with the admissions process, welcome new residents and families, and help ensure smooth, supportive transitions into the community.
Partner with physicians, nursing staff, and healthcare professionals to address the social and emotional factors impacting resident wellbeing and participate in care planning meetings.
Complete social histories, assessments, discharge planning, and required documentation accurately and in compliance with organizational and regulatory standards.
Assist families with Medical Assistance applications and connect residents with appropriate resources, services, and support systems.
Provide training and education for staff while maintaining positive relationships with residents, families, colleagues, and community partners.
Maintain resident confidentiality, uphold professional standards, and contribute to a compassionate, collaborative, and resident-focused environment.

Bachelor’s degree in Social Work (BSW) from an accredited college or university. One to two years of related experience, or an equivalent combination of education and experience. Strong interpersonal, communication, and relationship-building skills. Ability to effectively support residents, families, and interdisciplinary teams in sensitive and emotional situations. Computer proficiency and ability to complete documentation accurately and efficiently. Ability to interpret regulations, write reports, and communicate effectively with a variety of audiences.
Experience working with older adults in long-term care, healthcare, or senior living environments. Knowledge of geriatric mental health services and resident-centered care practices.
