Requires a Registered Nurse license in MN or WI and a nursing degree (BSN with 2+ years or ASN with 4+ years of community care experience). Preferred candidates have 5+ years of experience and proficiency with EPIC EMR.

The Referral and Scheduling RN is essential to the seamless coordination of referrals and scheduling across Home and Community Services, supporting a broad range of programs including hospice, home care, palliative care, and other specialized community services. This role manages the referral through admission, and scheduling processes ensuring patient visits are scheduled appropriately to maintain manageable caseloads and high-quality care. The RN collaborates closely with interdisciplinary teams, coordinates coverage when team members are unavailable or require visit support, and serves as a key resource for clinical, reimbursement, and regulatory matters. Compassionate communication, strong organizational skills, and a commitment to supporting patients, families, and staff are central to success in this role.
This role will focus primarily on supporting Hospice referral and scheduling.
• MN and WI RN Licensure
• Experience with the EPIC EMR setting
