Responsibilities

• Provide Physical Therapy services under supervision per plan of care for LTAC residents
• Assess and re-evaluate functional status; document progress and adjust interventions
• Lead therapeutic exercises, gait training, ROM, balance activities, ADLs, and transfers
• Apply modalities and manual techniques per protocol; ensure patient safety and comfort
• Collaborate with PT, nursing, rehab, and case management; participate in rounds and care planning
• Maintain accurate records in the EMR; uphold infection control, safety, and privacy standards

Requirements

• Active Physical Therapy Assistant license in New Jersey or eligibility for licensure
• Associate degree from an accredited PTA program
• Ability to work a 5x8 schedule and travel to assignment as required
• Experience in LTAC or skilled nursing settings preferred
• Strong communication and teamwork skills; reliable transportation to the assignment
