Overview Physical Therapy Assistant – LTAC in Fair Lawn, New Jersey. This travel assignment starts on 07/20/2026 for 13 weeks, with guaranteed 35 hours per week. Work a 5x8 day schedule (08:00–16:30) in a skilled nursing facility with long-term acute care needs. This role centers on delivering rehabilitative services to residents under supervisory guidance, supporting mobility, independence, and functional outcomes.

Responsibilities

Provide Physical Therapy services under supervision per plan of care for LTAC residents
Assess and re-evaluate functional status; document progress and adjust interventions
Lead therapeutic exercises, gait training, ROM, balance activities, ADLs, and transfers
Apply modalities and manual techniques per protocol; ensure patient safety and comfort
Collaborate with PT, nursing, rehab, and case management; participate in rounds and care planning
Maintain accurate records in the EMR; uphold infection control, safety, and privacy standards

Requirements

Active Physical Therapy Assistant license in New Jersey or eligibility for licensure
Associate degree from an accredited PTA program
Ability to work a 5x8 schedule and travel to assignment as required
Experience in LTAC or skilled nursing settings preferred
Strong communication and teamwork skills; reliable transportation to the assignment
