The RN Care Manager provides patient-centered care coordination within the Emergency Center and inpatient settings to facilitate safe transitions of care. They collaborate with interdisciplinary teams to identify barriers to discharge and implement appropriate interventions for patients and their families.
Candidates must hold a current Minnesota Registered Nurse (RN) license, with a Bachelor's degree in Nursing strongly preferred. Experience in hospital-based med-surg nursing and specialized areas like utilization or case management is highly desired.

The RN Care Manager provides patient-centered care management services to patients and families within both the Emergency Center and inpatient settings. This position serves as an integral member of the interdisciplinary healthcare team and will be responsible for identifying care coordination needs in collaboration with the inpatient and ambulatory care teams, patient and patient’s external support system (as applicable). Within the context of an interdisciplinary team, the RN Care Manager will participate in progressing patient towards discharge/next level of care by identifying barriers and implementing appropriate solutions/interventions. The RN Care Manager will work with patients and patients’ external support system to secure appropriate and safe transition plans in keeping with shared goals of care. The RN Care Manager will consistently and thoughtfully apply nursing values and continuous quality improvement in daily work.
