The Social Worker conducts psychosocial assessments and coordinates comprehensive social work services for patients in a rehab unit. They act as a liaison between patients and community resources to facilitate transitions of care, including nursing home and hospice placements.
Requires a Master's degree in Social Work and a current Alabama Social Workers license (LGSW required, LCSW preferred). Candidates must possess or obtain Basic Life Support Certification and preferably have six months of acute care experience.
The Social Worker completes psychosocial assessments and participates in treatment team meetings; provides individual and/or family conferences; screens patients on admission for transition of care; acts as a liaison with community resources/agencies: reports abuse and neglect to DHR; maintains a current list of community agencies/resources; assigns primary care physicians as needed; links patient with community resources i.e. nursing home placement, home healthcare, hospice, transportation, as well as plans, organizes, coordinates, and implements comprehensive social work services to assigned patients.

Master's degree in social work from an accredited college or university and a current Alabama Social Workers license. Minimum of six months acute care experience preferred. LGSW required, LCSW is a plus. Must possess Basic Life Support Certification by the American Heart Association or obtain with 30 days of hire.
