Coordinates pharmaceutical care by collaborating with interprofessional teams to optimize safe medication use. Responsibilities include reviewing medication orders, managing drug therapy protocols, and overseeing an outpatient anticoagulation clinic.
Requires a Doctor of Pharmacy degree and a current state Board of Pharmacy license. BCLS certification is required, while ACLS is preferred.

Coordinates and provides pharmaceutical care for patients, collaborating with the interprofessional team to optimize safe, effective medication use. Reviews and verifies orders, manages drug therapy protocols, educates staff/patients, and supports quality and regulatory standards. Reports to the Director or Manager of Department.
Knowledge, Skills & Abilities
Doctor of Pharmacy.
Current license with the state Board of Pharmacy.
BCLS required; ACLS preferred.
Critical thinking, decisive judgment, and ability to work with minimal supervision in stressful environments. Rapid learning and navigation of multiple pharmacy/clinical software systems. Effective English communication (read, write, speak, and understand). Ability to complete the Skills Competency Checklist within 90 days of hire.
