The Case Manager will provide intensive community-based services to seriously and persistently mentally ill consumers, facilitating service linkage across mental health and non-mental health systems. Responsibilities include direct support like transportation, coordinating team meetings, aggressive advocacy, and maintaining accurate electronic consumer clinical records.
Candidates must possess either an MSW or a Master's degree with clinical training, or a Bachelor's degree in a related field plus one year of relevant post-degree experience. A valid driver's license in good standing is also required, along with knowledge of mental health symptoms and available treatment resources.

Oaks Integrated Care is actively seeking for a qualified Case Manager for our ARRIVE program in Lumberton, Burlington County NJ.
The Case Manager will have the ability assess and interpret information related to population; have knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; have a general understanding of treatment needs; knowledge of available treatment resources and appropriate use; Population-specific communication skills.
Schedule: Mon-Fri 8:30 to 5 or 9 to 5:30 with some evenings and weekends as needed; 30 minute unpaid lunch
