The Case Manager will provide assistance to families for housing needs and address all identified life domain needs through outreach and service provision. Responsibilities also include participating in treatment plan development, maintaining provider contacts, and completing all required documentation and training.
Candidates must possess a Bachelor's degree in social work, psychology, or a related field, along with required social service experience. A valid driver's license in good standing and passing a pre-employment drug screening are mandatory for all positions.

As a Case Manager, you will work in-home and community based setting in Moorestown, Burlington County, NJ.
Schedule: 40 Hours Monday-Friday, varied hours, some evenings
