The Case Manager is responsible for providing supportive services to residents, including developing individual service plans and conducting regular unit inspections. They also facilitate health-promoting groups and maintain a liaison with community services to ensure resident well-being and self-sufficiency.
Candidates must hold a Bachelor's degree in Social Work or a relevant field and possess at least three years of post-bachelor experience. A working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness is required, along with strong interpersonal and computer skills.

Join The Doe Fund on our mission to break the cycles of homelessness, addiction, and recidivism. Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs.
We are seeking a Case Manager for one of The Doe Fund's permanent supportive housing for homeless individuals and families living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being.
The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population.
