The Peer Support Worker coordinates patient care within an integrated health team, assessing biological, social, and emotional factors to develop holistic care plans. They provide behavioral interventions, crisis triage, and assist patients in navigating community resources and financial assistance programs.
A high school diploma or GED is required, with a Bachelor’s degree in a human services field preferred. Candidates must have at least three years of specialized experience in a related field, preferably within a primary care or behavioral health setting.
The Peer Support Worker coordinates patient care with members of the integrated health care team. They assess, within the framework of a holistic approach, the biological social, emotional, familial, and financial factors impacting the patient/family’s ability to cope with problems of daily living related to their mental health/chronic disease condition. They collaborate with cross-departmental teams and/or members of partner agencies (schools, DCF, mental health and substance misuse providers) to develop care plans.
This position has no direct supervisory responsibilities.
