The Nurse Navigator coordinates care for breast cancer patients from diagnosis through the entire care continuum. Responsibilities include assessing physical and psychosocial needs, providing patient education, and collaborating with interdisciplinary medical teams.
Requires a BSN, a Minnesota nursing license, and 2-3 years of experience in oncology patient care. Candidates must have BLS provider status and the ability to obtain OCN certification within 2-5 years.

The Nurse Navigator in Breast Health is a nursing practice role serving patients with breast cancers and their families throughout the Park Nicollet integrated health system. This role identifies patient needs from diagnosis through the care continuum, coordinates care and provides consultation and collaboration with interdisciplinary teams (including surgery, oncology, radiation therapy, radiology, and pathology). Responsibilities include assessment, intervention, ongoing evaluation of physical, psychosocial, and financial needs, patient/family education, and resource identification.
